Glaucoma Treatment Evolution: A Journey Through Decades

During the 1900s the treatment choices available, for glaucoma were quite basic. Specialists primarily concentrated on lowering the pressure inside the eye, known as pressure (IOP) which was a factor contributing to glaucoma advancement. Surgical procedures, usually performed in eye hospitals like iridectomy and trabeculectomy were aimed at enhancing the drainage of fluid, from the eye. However, these interventions were invasive carried risks and often resulted in long term effectiveness.

The Rise of Pharmaceuticals: From Beta Blockers, to Prostaglandin Analogues

In the 1970s there was a shift, in glaucoma treatment, with the introduction of drugs. Eye care specialists started prescribing beta blockers like timolol, which revolutionized treatment by reducing the production of humor and lowering IOP (pressure). These medications provided an option compared to surgery and became an essential part of glaucoma therapy.

In the following decades prostaglandin analogues, like latanoprost were developed. These medications improved outflow adding diversity to the available treatment options. The topical nature of these drugs made them highly effective, well tolerated and easy to administer leading to compliance and improved outcomes observed in eye care centers.

Laser treatment has made progress transitioning from the use of argon lasers to an advanced technique known as Selective Laser Trabeculoplasty (SLT).

Laser technology is currently being employed as a method to treat glaucoma. In the 1980s a technique called argon laser trabeculoplasty (ALT) was introduced as a method to enhance the movement of aqueous fluid through the trabecular meshwork by employing photocoagulation. While ALT proved effective it had some side effects and limited ability for repeated treatments.

Moving into the century we witnessed a groundbreaking development known as laser trabeculoplasty (SLT). This new technique specifically targets pigmented cells in the trabecular meshwork. Has shown effectiveness to ALT but with fewer complications. Moreover, SLT allows for repeat treatments making it more valuable, in managing glaucoma in eye care centers.

Redefining the Treatment Paradigm; Exploring Microinvasive Glaucoma Surgery (MIGS)

The search, for ways to treat glaucoma has resulted in the development of a groundbreaking approach called glaucoma surgery (MIGS). Unlike glaucoma surgeries MIGS procedures performed by eye surgeons are well known for their effects, on the tissue fast recovery time and reduced risk.

Innovative devices such as the micro bypass stent and the Xen gel stent exemplify the nature of MIGS offering long lasting reduction in intraocular pressure (IOP) while preserving the structure of the conjunctiva. MIGS has successfully bridged the gap, between therapy and traditional surgery empowering eye surgeons with a range of options tailored to meet individual patient needs.
